Current Patients
Many IVF facilities agree to store their patients’ cryopreserved specimens for one year or less, and then require that the specimens be moved to RTL for long term storage. After obtaining patients’ documentation to continue storage of their specimens at RTL, you forward patients’ names, addresses and phone numbers to RTL. We contact each patient and send them the forms required for specimen transfer. We update you regarding receipt of forms from these patients.
Once the accounts of several patients are complete, we contact you to arrange the transfer of specimens for the entire group. By completing a group transfer, the shipping fees ($300 for two tanks) are divided among all the patients being transferred. If the clients have more than one straw/vial of cryopreserved embryos, we use two shipping tanks and ask you to split the patients’ specimens, if possible, as an added insurance against tank failure. We have an additional quality control test that is completed on every shipment to document shipping tank conditions.
New Patients
We can also supply you with sets of our forms that can be given to your new patients for completion during their initial appointments prior to their IVF procedure. You would then forward the previously completed RTL forms to us when you are ready to transfer a patients specimens to RTL for long term storage. In that situation, you would alter your consents to allow for an automatic transfer of specimens to RTL after a specific time (one year or less). Upon receipt of the forms, we set up an account for your patients.
Once the accounts of several patients are complete, we contact you to schedule the transfer for the entire group following the procedures listed above.
