To Transfer Specimens to RTL for Storage
To transfer semen, embryo, or oocyte specimens to RTL for storage, patients are required to complete paperwork to open an account at RTL and to authorize the transfer. In addition, patients must provide serology test results for HIV 1&2 (AIDS). The fees for the transfer and storage must be prepaid by Visa, MasterCard,
Discover, American Express, check or money order.
Once the above requirements have been completed, RTL staff will contact the patients clinic to arrange for the transfer. RTL
recommends the use of two transport shippers for the transfer if there is more than one straw or vial of embryos, semen or oocytes being transferred. RTL documents the successful transfer through the use of a temperature exposure indicator, which monitors the
transport shipper's environment throughout the shipping process.

To Transfer Specimens from One Facility to Another Facility
To transfer semen, embryo, or oocyte specimens from one facility to another facility, patients are required to complete paperwork to open an account at RTL and to authorize the transfers. These transfers are done in two steps, with the specimens coming to RTL from the first facility and then transferred to the second facility in a separate shipment. Patients must provide serology test results for HIV 1&2 (AIDS), HBsAg, Anti-HCV and HTLV I&II. Patients must pay for a minimum of one quarter storage, even if the specimens are only at RTL for a short period of time. The fees for the two separate steps of the transfer and a minimum of one quarter storage must be prepaid by Visa, MasterCard,
Discover, American Express, check or money order.
